This is where your journey with Adaptive SDP officially starts. We’ve outlined the simple steps below to help you move forward, from submitting your interest to meeting your care team and launching services.

Step 1:
Complete Your Interest Form

This is your first (and most important) step. Filling out our short Interest Form is how you take action toward meaningful change. It lets us know you’re ready to improve your life and begin building the kind of support that works for you.

Step 2:
Meet With Our Team

After receiving your form, we’ll schedule a virtual meeting to connect. We’ll talk about your goals, walk through how our services work, and begin learning more about what’s important to you. This conversation helps us understand how to best support your family moving forward.

Step 3:
Complete Intake & Assessment

We’ll send you a detailed intake form and schedule an in-home or virtual assessment. These steps help us learn more about your loved one’s routines, preferences, and environment. Everything we gather is used to create a plan that truly fits your needs.

Begin Services!

Once everything is in place, your services officially begin. We’ll finalize your care plan, confirm your schedule, and introduce you to your support team. From here on out, we’ll walk alongside you to make sure everything runs smoothly.
